Great programs already exist. The problem isn’t access — it’s follow-through.

Referrals break down in the field because no one closes the loop. That’s the gap we solve with a repeatable field skill: Community Health Detailing (CHD). CHD brings lived experience and trusted community relationships into clinical care using a proven educational outreach approach to inform and influence busy care teams.

  • Helps care teams better identify eligible patients, make referrals, and support enrollment into available programs and services.

  • Equips community-based workforce with the skills and strategies to share key messages, build trust, and influence referral behavior in busy care settings.

  • Maximizes existing investments by helping organizations get more value from clinical-community partnerships by increasing throughput to the programs and resources people need.

Program Structure

Our program is designed as a single, continuous Accelerated Learning Collaborative — moving teams from preparation to real-world practice in just a few months.

  • Month 1: Engagement Preparation

We start by equipping leads with the tools and clarity needed to lead successful implementation. This includes: two live, manager-only sessions, program configuration and planning, and building coaching skills to support detailers

  • Month 2: Skill Building & Practice

Program participants build and refine core skills in a structured, cohort-based environment before entering the field. This includes: four live sessions (90 minutes each) over 2-3 weeks, along with guided practice with real-time feedback and coaching

  • Month 3-4: Real-World Practice

Participants apply skills in the field, supported by ongoing coaching and peer learning. This includes: four live coaching sessions, real-time troubleshooting and peer learning, and continued skill development based on field experience

Testimonials

“This project allowed our Ys to rethink the way they approach their relationships with healthcare providers…it provided [our Ys with] a tool that allowed them to effectively communicate what they were able to offer during this extra difficult time, especially those suffering from chronic diseases.”
